Step 1. Confirm your Eligibility Requirements

  • Age 0 – 65
  • Australian Citizen, Permanent Resident or hold a permanent/protected Visa
  • A formal diagnosis from a treating specialist

Step 2. Prepare all your Supporting Evidence from your Treating Specialist

  • Diagnosis of your disability with completed assessment(s) from your treating specialist
    • Date of diagnosis
    • What supports are recommended to support you with your disability
  • Diagnosis of your disability with completed assessment(s) from your treating specialist
    • Date of diagnosis
    • What supports are recommended to support you with your disability

Step 3. Contact the Brotherhood of St. Laurence (BSL) or Latrobe Community Health Service

  • You will be connected to a Duty LAC who will:
    • Guide and assist you with the next steps
    • Booking in a meeting to complete your NDIS application
